Hometown Healthcare Integrated Delivery GCHS Hospital, Greeley County Family Practice Clinic, and Long Term Care are all connected in the same building complex. This allows us to provide seamless care across all of our service lines. We are privileged to provide ambulatory care at the Wallace County Family Practice Clinic in Sharon Springs. Wallace County Hospital State-of-the-art software, equipment and an impressive suite of services assist our top-notch staff in providing outstanding patient care at this 18-bed critical access hospital. -- Clinics GCHS provides ambulatory care in both Greeley and Wallace counties. Three providers, five mid-levels, and an excellent nursing and administration staff ensure that you and your family will receive exceptional care. -- Long Term Care Attached to the Hospital and Clinic complex, our 32-bed long term care facility gives residents immediate access to the staff and services at GCHS. The recently remodeled building boasts wonderful accommodations in a comfortable layout. -- Community Centered Greeley County Health Services is committed to the growth and stability of the communities that we serve. Construction is moving along smoothly with new infrastructure and equipment being installed daily. The CHNA identifies the problems, leads to the development of targeted action plans and suggests placement of resources where they are most needed and effective.
